Assessments

Prior to entering the support groups, families participate in an assessment process.

The assessment process begins with a scheduled phone call from the caregiver. The purpose of this time is to gather information regarding the child’s experience with the death and observations following the death.

Upon completion, an assessment appointment is scheduled. This time is arranged for the caregiver and child(ren) to be seen at Hearts and Hope during regular business hours. This opportunity is provided for the family to learn more about the groups at Hearts and Hope and what it would be like to participate. They are encouraged to share their story about the loss - thoughts, feelings, experiences, questions, & memories.
The assessment is also provided so the adult caregiver and the child(ren) can meet individually with a Program staff member who will determine where each is in his/her grief process and make sure that the support offered through our groups is adequate to meet the needs of each person. When needed, recommendations and referrals for additional support services are provided.

Supporting non-English speaking families
All appointments are conducted in English and must be scheduled with Program staff. If helpful, a translator can participate in the assessment process. The family should identify an adult who can assist (the child cannot serve as the translator). The initial telephone appointment may be conducted in person if it better meets communications needs.
